Freedom of Information Act Request
Overview
Requests for public records are fulfilled under the guidelines of the Illinois Freedom of Information Act (5 ILCS 140). Tracy Miracle, Administrative Analyst is the appointed Freedom of Information Act (FOIA) Officer for the Village of Beach Park. The FOIA Officer is located in the Administration Office of the Village Hall, 11270 W. Wadsworth, Beach Park, IL. 60099. FOIA requests can be submitted via e-mail.
How to Request a Record
All requests must be submitted in writing to the attention of the Freedom of Information Act Officer. Requestors may use the Village of Beach Park’s suggested FOIA Request Form or may submit their own written request for records. Requestors choosing not to use the provided form should provide their full name, email or street address, phone number, a detailed account of the documents they are requesting and whether or not they are requesting the documents for commercial use.
Written requests may be submitted in person at the Administration Office at Village Hall, mailed or e-mailed to the attention of the Freedom of Information Act Officers, or faxed to 847-746-1797. Requestors will be contacted within the time allotted by law with a response to their request; at that time the requestor will be informed of any fees associated with their request.
Certified Documents
For an additional charge documents may be certified. Certified documents have a signed and sealed letter from the Village Clerk stating that the attached documents are true and correct copies of the original documents on file with the Village of Beach Park. The Clerk’s Office advises those needing public records for legal proceedings to have their documents certified.
Please note that the requestor will be notified if any records they have requested need to be sent out for reproduction/printing. The requestor will be charged at cost for said reproductions/printing jobs.
